This is also available as a single PDF (Additional File 9). *The total number of articles for each full-text exclusion criterion includes multiple reasons allocated to a single article; we also expected that when articles failed to meet multiple exclusion criteria, screeners may not have indicated every reason for exclusion (e.g. if the article was the wrong article type)
